Update on the Management of Aging Women Undergoing IVF-ET
The most dominant criteria for Poor ovarian response (POR) during stimulation for IVF are small numbers of follicles retrieved, which mostly due to advanced maternal age or diminished ovarian reserve.
- Although several strategies have been proposed for clinical management, the results have been remained challenging
- The key point for aging women is to get more good quality oocytes and embryos.
- The challenge in ART management for aging infertility women is the quantity and quality of oocyte.
- Luteal-phase ovarian stimulation（LPOS）, progestin-primed ovarian stimulation（PPOS）or Double stimulation(Shanghai) protocol can be proposed for women with diminished ovarian reserve with the advantages of more cost-effective and patient-friendly, more options and chances to harvest oocytes and available embryos at the limitation of ovarian reserve.
- Supplementation of mitochondrial nutrients, such as growth hormone , coenzyme Q10, resveratrol , etc. may lead to improvement in oocyte and embryo quality for ageing women.
- TCM has a long history of treating infertility with theory system in China. Its application has showed that “Shen-nourishing” TCM may improve the positive outcome in aging patients undergoing IVF-ET including increasing pregnancy rate and decrease OHSS.
- The result of our multiple-center, randomized , placebo control clinical trial with ZISHEN YUTAI pill provide with the A level evident-base reference to its application.
