Abstract
The Autoimmune Bases of Infertility and Repeated Implantation Failure After IVF/ICSI-ET
Recurrent implantation failure (RIF) after IVF-ET and recurrent pregnancy loss (RPL) remains a common problem, which occurs in 1%-5% of couples of reproductive age. The aetiologies of RIF and RPL include maternal imbalances immune system. However, the nature of immune alterations in RIF and RPL remains ill-defined with limited evidence for suitable immune biomarkers to identify patients that may be suitable for immunomodulating treatment. Aspecific auto-immunity defined as positivity of auto-antibodies in blood sample without clinical or biological criteria for defined diseases. The presence of anti-thyroperoxydase antibodies and/or throglobulin are related to infertility; anti-phospholipid (APL) and/or anti-nuclear (ANA) antibodies are more frequent in the population of infertility women. Current treatment modalities include immunomodulating and immunosuppressive therapies. But there are no large randomised controlled trials to deliver high-level evidence of clinical benefit. We examine the currently available evidence for immunological focussed laboratory testing to guide treatments for RIF.
